Responsibilities
- Provide personal care assistance including bathing, grooming, dressing, and mobility support using patient transfer methods such as Hoyer lifts when necessary.
- Assist with Activities of Daily Living (ADLs), ensuring clients' needs are met with dignity and respect.
- Monitor and document vital signs, medication administration, and changes in health status accurately within Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems.
- Support patients with specialized needs such as Alzheimer’s care, dementia care, or memory care by implementing tailored approaches to ensure safety and comfort.
- Maintain patient confidentiality in accordance with HIPAA regulations while communicating effectively with clients, families, and healthcare teams.
- Assist with light housekeeping duties including cleaning, laundry, meal preparation, and ensuring a hygienic environment.
- Follow home healthcare regulations and safety protocols during patient transfers or when operating medical equipment like Hoyer lifts or other mobility aids.
- Respond promptly to emergencies by applying first aid or basic life support (BLS) techniques such as CPR when required.
- Support clients through end-of-life or hospice care with compassion and professionalism.
- Maintain thorough medical documentation of care provided and ensure compliance with all healthcare standards.
Experience
- Prior experience working in assisted living facilities, nursing homes, home healthcare agencies, or hospice environments is preferred.
- Demonstrated ability to work with individuals with developmental disabilities or those requiring specialized dementia or Alzheimer’s care.
- Familiarity with medical terminology, medication administration procedures, and vital signs monitoring is essential.
- Experience utilizing EMR/EHR systems for documentation and patient records management.
- Knowledge of home health care assistance techniques including patient transfer methods such as Hoyer lifts and safe lifting practices.
- Proven ability to handle heavy lifting safely while assisting patients with mobility challenges.
- Strong background in geriatric or senior care along with caregiving skills that promote independence and dignity for patients of all ages.
- Certification in first aid and CPR is required; additional certifications related to hospice or palliative care are advantageous.
