What are the responsibilities and job description for the Behavioral Health Case Manager position at Omaha Tribe of Nebraska?
SUMMARY: The Behavioral Health Case Manager's primary duties involve connecting individuals with appropriate mental health services and resources, developing individualized care plans, and ensuring clients receive the necessary support to improve and maintain their mental health and well-being. This position advocates for clients' rights, provides crisis intervention, and monitors progress towards treatment goals and maintenance.
ESSENTIAL DUTIES and/or RESPONSIBILITIES include the following:
- Assist primary health care providers (doctors, nurses, ambulance, Tribal Court, public schools, local Law Enforcement) in recognizing and treating mental health, substance abuse and resulting psychosocial problems.
- Serve as public guardian for identified vulnerable adults in need of assistance per court involvement.
- Collects data about patients through interviews, case history and/or observational techniques; evaluates data to identify causes of problems and refer to specialists.
- Establishes and maintains case files, referrals and other related documents for the treatment of clients; prepares related documentation for civil commitment hearing or other legal proceeding; keeps well-informed of patient data to ensure appropriate treatment and care are delivered.
- Develops and maintains relationships with all agencies who provide services to Native Americans in the area of Behavioral Health.
- Provide guidance, support, and encouragement to clients throughout their treatment journey.
- Work collaboratively with other healthcare professionals, social workers, and family members to ensure coordinated care.
- Conduct follow-up visits and provide ongoing support to ensure continuity of care.
- Manage various aspects of a client's case, including paperwork, appointments, and follow-ups.
- Educate clients and families about mental health conditions, treatments, and available resources.
- Provides patient assessment, under the diagnosis from the provider for patient treatment plans in accordance with statutes, regulations and protocols regulating the profession.
- Collaborates with mentors, colleagues, clinic providers, the nursing staff and community outreach personnel, as well as the counselors at the Carl T Curtis Health Education Behavioral Health department.
- Provides psychiatric health services, education, counseling and emotional support.
- Issues medication prescriptions in accordance with treatment guidelines.
- Assist patient in following up with orders such as laboratory tests, medications, and injections.
- Transcribing chart notes and medication refills for psychiatric provider.
- Maintain accurate and up-to-date case records.
- May also provide primary medical care services as indicated, in accordance with statutes, regulations and protocols regulating the profession.
- As needed, performs some administrative function such as preparing reports and correspondence and may propose changes to program policies and procedures.
- Preforms miscellaneous job-related duties as assigned.
EDUCATION and/or EXPERIENCE:
- Certified Medical Assistant Program
- 5 or more years of experience related to the above-described duties.
CERTIFICATES, LICENSES, and/or REGISTRATIONS:
- Certified Medical Assistant
- Must be credentialed per Carl T. Curtis Health Education Center Behavioral Health Department policies and procedures of billing of services under Medicare/Medicaid.
- Possess a valid Driver's License.
- Must have a clean driving record and be insurable under the Health Center's insurance.
- CPR/AED. BLS Certified
- Mandatory Reporter Certified
