Premiums
Provides specialized occupational therapy services to primarily adult and geriatric patients in their homes. The role involves a range of responsibilities focused on improving the patient’s overall quality of life, maximizing their functional abilities, ensuring home safety and promoting independence. This will include conducting comprehensive patient assessments, developing individualized treatment plans, implementing therapeutic interventions and providing home safety recommendations for overall improved patient quality of life and independence as much as possible. Consult and coordinate with home health interdisciplinary team and other medical professionals regarding the patient’s comprehensive plan of care.

Premiums
- Weekend - $4.00/hour
- Lead - $3.25/hour
- Preceptor - $1.00/hour
- Per Diem - 15% (on base plus steps, in lieu of benefits)
- Certification - $2.00/hour
- Weekend Coordinator Pay (chg) - 15% on regular pay

EDUCATION
- Bachelor’s or master’s degree in occupational therapy from an accredited program required.
- One-year Occupational Therapy experience providing rehabilitation therapy to a variety of patients including the elderly required.
- Experience in home health or geriatric setting preferred.
- Licensed as an Occupational Therapist in the state of Washington required.
- Current BLS certification required.
- Valid Washington State driver’s license required.
- Proof of insurance on automobile used for work required.
- Driving record in compliance with OMC standards currently in effect or as may be amended, required. See HR Policy Driving for OMC Business.
- OMHH issued work cell phone provided.
- Must have computer internet connectivity (OMHH work laptop provided).
- Must have reliable automobile transportation to travel to patient residences.
