Role Description
We are seeking a part-time Nurse Practitioner for a hybrid role based in Pinellas County, FL. This position involves providing primary and transitional care to patients in ALFs, group homes, private residences. Responsibilities include conducting patient assessments, developing comprehensive care plans, monitoring chronic conditions, and coordinating with healthcare partners for lab, radiology, and specialist services. This role allows for a combination of on-site visits and remote work to ensure excellent patient care and flexibility for our team members.
Qualifications
- Strong clinical skills in primary care, chronic care management, and transitional care
- Proficiency in patient assessments, diagnosis, and individualized care plan development
- Ability to collaborate with multiple healthcare partners including labs, specialists, and home health services
- Excellent communication and interpersonal skills to foster patient relationships and coordinate care among teams
- Experience with telehealth platforms and technology
- Current Nurse Practitioner license in the state of Florida
- Ability to work both independently and collaboratively in a hybrid work environment
- Previous experience in mobile healthcare delivery, geriatrics, or family practice settings is a plus
