Community Health Worker

Qualifications:

• High School Diploma or equivalent required.
• Minimum one (1) year experience working with HIV populations preferred.
• Willing to be stationed and travel to clinics with clients in the 6 counties we service that are Oakland, Macomb, Wayne, St. Clair, Lapeer, and Monroe.
• Must obtain Michigan Community Health Worker certification within one year of hire.
• Computer literacy and comfort with various forms of data tracking software.
• Valid Michigan Driver’s license; active insurance and reliable transportation to be used for work related travel required.

Job Mission:

To connect newly diagnosed or out of care individuals living with HIV to medical care

and support services.

Performance Standards:

• Perform outreach to identify People Living with HIV/AIDS (PLWHA) and provide linkage to care and health education.
• Develop relationships with federally defined points of entry to market services and facilitate referrals.
• Participate in quality improvement initiatives for assigned programs
• Provides health education and risk reduction counseling, information about available resources and services, and psychosocial support to clients as they access the continuum of care.
• Carry out all other duties as assigned by supervisor.
