Duties
- Conduct comprehensive patient assessments, including vital signs, physical examinations, and detailed health histories to inform individualized care plans.
- Administer medications and injections accurately, including IV infusions and medication management, following strict aseptic techniques.
- Perform specialized procedures such as suturing, catheterization, tube feeding, and wound care with precision and adherence to safety protocols.
- Utilize electronic health record (EHR) systems like Axxess to document patient information, treatment plans, and medical records efficiently.
- Collaborate with interdisciplinary group to provide holistic hospice care.
- Manage complex cases involving infectious disease care or hospice & palliative medicine.
- Support patient education on health coaching topics such as chronic disease management, medication adherence, and lifestyle modifications.
- Ensure compliance with HIPAA regulations and maintain the highest standards of infection control and patient safety throughout all procedures.
Experience
- Proven clinical experience in acute care environments such as hospital medicine, home health, or emergency rooms.
- Prior work in diverse settings such as medical-surgical units, outpatient clinics, long-term care facilities, assisted living communities or home care environments.
- Familiarity with EMR/EHR systems for accurate documentation and patient tracking.
- Experience working with individuals with disabilities or developmental disabilities is highly valued.
- Strong understanding of physiology knowledge for accurate patient assessment and diagnostic evaluation across various specialties including geriatrics and pediatrics.
