Our RNs provide skilled nursing visits, patient assessments, care coordination, medication education, wound care, disease management, post-hospital follow-up, and support for patients recovering from illness, surgery, injury, or a change in condition.

- OASIS documentation and care planning
- Medication reconciliation and patient education
- Wound care and chronic disease management
- Communication with physicians, families, therapists, and the care team
- Helping patients remain safe, independent, and confident at home
Our LPNs provide hands-on skilled nursing care under the direction of the RN and physician plan of care. This role is ideal for a nurse who enjoys patient education, strong clinical follow-up, and building relationships with patients and families in the home setting.
LPN responsibilities may include:
- Skilled nursing visits in the patient’s home
- Medication education and treatment follow-up
- Wound care and vital sign monitoring
- Support for chronic disease management
- Documentation of patient progress and changes in condition
- Communication with the RN case manager and clinical leadership team
- Helping patients stay safe and supported at home

- Strong communicators with patients, families, and providers
- Organized and timely with documentation
- Passionate about helping patients recover and remain safely at home

- Active Virginia RN or LPN license
- Current CPR certification
- Reliable transportation
- Valid driver’s license and auto insurance
- Home health experience preferred, but we are willing to train the right candidate
- OASIS experience preferred for RN applicants
