Old Colony Hospice is seeking a full-time day shift Registered Nurse Case Manager for the South Shore from the Braintree area to the Kingston area, including Hingham, Cohasset, Scituate, Marshfield, Duxbury, Pembroke, Norwell, Hanover and more. The Hospice RN Case Manager coordinates hospice services for the assigned caseload of patients/caregivers collaborating with an interdisciplinary team (IDT) and the patient’s primary physician in order to maximize the comfort and health of patients, families, and caregivers regardless of the care setting, to promote continuity and adherence to the hospice plan of care.
ESSENTIAL DUTIES:
- Manage a caseload consistent with industry standards being competent in end-stage disease symptom management and act as an advocate and case manager for patients, families and caregivers to assure that the choices of the patient and family/caregivers are honored
- Inform primary physician and/or medical director and other personnel of changes in the patient’s needs and outcome of an intervention
- Coordinate all patient/family services and prioritize needs with the members of the IDT; consult with and educate the patient/family/primary caregiver regarding disease process and care techniques
- Provide specialized pain/symptom management related to end of life care
- Determine the frequency of visits based on acuity and level of care needs
- Maintain and complete all required computerized and paper documentation in accordance with any applicable agency or federal or state regulations
- Participate in process improvement activities and team meetings as requested
- Share in providing on-call/continuous care coverage as may be needed, completing necessary documentation
Qualifications:
- Licensed to practice as an RN in the Commonwealth of MA
- Driver's License (required)
- 1-Year Nursing Experience (preferred but not required)
- Ability to use good judgment and discretion and to work effectively as a member or part of an interdisciplinary team
- Must have knowledge and understanding of applicable federal, state, and local hospice, Medicare, Medicaid, HMO regulations, as well as HIPAA and other regulations protecting personal health information of patients.
- Must be able to maintain reliable attendance and be flexible.
