Summary
Collaborate with team members to improve health outcomes for high-risk populations.
ESSENTIAL FUNCTIONS AND RESPONSIBILITIES
- Cross reference clinical databases to identify high-risk populations
- Participate in quality data collection for programs focused on improving health outcomes
- Communicate with clinical team regarding high-risk population needs and engagement
- Facilitate connecting high-risk populations with appropriate community resources.
- Document interactions and collaborative efforts in electronic medical record
- Help with needed wellness services for clients when requested
- Assist with staff wellness when requested
- Attend required clinical meetings prepared with information to advocate and benefit high-risk populations
- Attend staff meetings, in-service training, workshops, and seminars and adhere to agency policy, rules, regulations, and procedures
- Performs other duties as required
QUALIFICATIONS
- High School Diploma or General Equivalency Diploma required
- Bachelor’s Degree in mental health or health-related field preferred.
- Minimum two years relevant experience required
- Oklahoma Department of Mental Health and Substance Abuse Certified Case Manager I, if eligible, and Wellness Coach required within 90 days of hire required (training provided after hire)
- Peer Recovery Support Specialist certification, if applicable, preferred
- Valid OK driver’s license, clean driving record, reliable transportation required
- Excellent verbal, written and electronic communication skills required

PHYSICAL REQUIREMENTS
- Must have the ability to stand or sit for long periods of time
- Must have the ability to lift, push, or pull a minimum of 25 pounds
- Ability to travel approximately 10% of the time
- Ability to use telephone, PC, fax machine, copy machine, and printer
