Position Summary

The Oklahoma Hospital Association (OHA) Foundation’s Rural Health Transformation Program (RHTP) Coordinator supports the day-to-day implementation of OHA’s Rural Health Transformation Program grant funded initiatives.

This role coordinates and provides technical support for two statewide grant funded initiatives supporting rural hospitals in advancing population health, preventive care, and care coordination, with 50% of staff time dedicated to each grant.

The core focus of OHA’s RHTP grant funded programs are integration of hospital community health workers (CHW) and lung cancer screening navigators (LSNs) in rural hospitals. This position will assist partner hospitals with patient referral processes, care coordination, data tracking, and follow-up related to health-related social needs (HRSN), tobacco use, and preventive lung health.

Primary Functions

· Coordinate day-to-day implementation activities for two RHTP funded initiatives.

· Provide ongoing, proactive technical assistance to RHTP-funded hospitals implementing CHW and LSN programs, including workflow development, problem-solving, implementation support, and identification of opportunities for improvement.

· Serve as a primary point of contact for assigned RHTP hospital partners, building strong, collaborative relationships and providing ongoing communication, coordination, and support.

· Support hospitals in translating RHTP program goals and requirements into practical, sustainable workflows and processes within their organizations.

· Monitor implementation progress across participating hospitals, identify areas needing additional support, and follow up with partners to promote timely completion of program activities and deliverables.

· Facilitate regular communication with hospital partners to share expectations, clarify program requirements, address questions, and connect partners with appropriate OHA resources and expertise.

· Support the development, implementation, and continuous improvement of program tools, resources, workflows, and training materials for participating hospitals.

· Identify and share promising practices, lessons learned, and successful strategies across participating hospitals to support peer learning and program improvement.

· Assist participating hospitals in integrating HRSN screening, tobacco cessation referrals, and lung cancer screening navigation into clinical workflows.

· Work with hospitals to ensure timely, accurate data submission in alignment with grant and evaluation requirements.

· Coordinate partner hospital meetings and trainings and support learning collaboratives.

· Collaborate with OHA communications staff to support dissemination of program updates and success stories.

· Coordinate program logistics, scheduling, documentation, data collection, and partner communications.

· Work with the Oklahoma State Health Department RHTP support team for applicable grant work plan activities.

· Stay current on best practices related to rural health, and best practices for HRSNs, lung cancer screening, and tobacco cessation.

· Support the OHA Foundation Executive Director and Director of Health Improvement Initiatives in applicable operations related to the RHTP programs for CHW’s and LSN’s.

· Coordinate with OHA Foundation staff for effective alignment of RHTP program strategies.

Qualifications

· Bachelor’s degree required in public health, community health, social work, nursing, or other health related fields.

· Minimum of 2–3 years of experience in public health, healthcare, or community-based programs.

· Experience working with community health workers, patient navigation, or care coordination preferred.

· Familiarity with rural health systems and underserved populations strongly preferred.

· Comfortable providing training, facilitation, and technical assistance.

· Strong written and verbal communication skills.

· Ability to travel in-state as needed.

Pay: $67,000.00 - $75,000.00 per year
