The Oklahoma Hospital Association (OHA) Foundation’s Rural Health Transformation Program

(RHTP) Manager provides strategic leadership and oversight for OHA’s Rural Health Transformation Program five-year grant funded initiatives.

This role is responsible for managing two statewide grant funded initiatives supporting rural hospitals in advancing population health, preventive care, and care coordination, with 50% staff time of staff time dedicated to each grant. and staff time will be split 50/50 between both grants. The RHTP Manager leads program strategy, oversees grant implementation, supervises the RHTP Coordinator, and serves as a primary liaison with the Oklahoma State Health Department, hospital partners, evaluators, and other stakeholders. The RHTP Manager works under the direction of the Director of Health Improvement Initiatives to support grant activities, assist hospital staff with program implementation and evaluation.

The core focus of OHA’s RHTP grant funded programs are integrating hospital-based community health workers (CHW) and lung cancer screening navigators (LSNs) in rural hospitals. This position will provide assistance to partner hospitals with patient referral processes, care coordination, data tracking, and follow-up related to health-related social needs (HRSN), tobacco use, and preventive lung health.

Primary Functions:

· Provide strategic leadership and oversight for the implementation and continuous improvement of OHA’s two RHTP grant funded programs.

· Supervise and guide the work of the RHTP Coordinator, ensuring program goals, timelines, and deliverables are met.

· Oversee program evaluation, performance monitoring, and grant compliance.

· Lead statewide strategy and program design for implementation of hospital community health workers and lung cancer screening navigators in rural hospitals funded through the RHTP grants, ensuring alignment with OHA priorities, funder requirements, and statewide rural health needs.

· Guide RHTP participating hospitals in health systems change related to preventive care, care coordination, and community-clinical linkages.

· Identify opportunities to scale, sustain, and replicate successful rural health models across Oklahoma.

· Build and maintain strong relationships with participating rural hospital leaders and frontline staff.

· Serve as a key point of contact with external partners, including the state health department, hospital partners, external evaluators, community-based organizations, and other stakeholders.

· Support onboarding and training of RHPT funded CHWs and LSNs to ensure effective program delivery in coordination with hospital leadership.

· Support RHTP funded health systems in workflow integration, patient identification, and referral processes for health-related social needs (HRSN), lung cancer screening, and tobacco cessation.

· Represent OHA at state and national conferences, coalitions, and workgroups to disseminate best practices and lessons learned.

· Monitor emerging research, evidence-based practices, and policy changes related to

HRSN, lung health, and tobacco treatment and integrate findings into program strategy.

· Assist participating hospitals in creating sustainability plans and building business cases for continued program support for post-grant funding.

· Support all contract monitoring procedures required by the OSDH

· Support the OHA Foundation Executive Director and Director of Health Improvement Initiatives in applicable operations related to the RHTP programs for CHW’s and LSN’s

· Coordinate with OHA Foundation staff for effective alignment of RHTP program strategies.

Qualifications:

· Minimum bachelor’s degree required; master’s degree preferred in public health, health administration, or other health related field.

· Minimum of 5 years of experience in public health, healthcare systems, population health, or rural health initiatives.

· Demonstrated experience managing complex, multi-site programs or grants.

· Strong understanding of rural healthcare systems, public health priorities, and health systems change principles.

· Experience working with rural and underserved populations preferred.

Skills and Abilities:

· Strong leadership, relationship-building, and facilitation skills.

· Ability to communicate effectively with executive leaders and frontline staff.

· Strategic thinker with strong organizational and problem-solving abilities.

· Comfortable working independently while collaborating across teams.

· Willingness and ability to travel in-state and occasionally out-of-state.

Pay: $67,000.00 - $85,000.00 per year
