Looking for a Physical Therapist to join our Home Health and Hospice start up in Oklahoma and surrounding areas

Responsibilities:
1. Evaluate and treat patients suffering from a physical disability due to injury, disease, or surgery in a home care environment
2.Establish treatment plans and treat patients using a variety of methods, including exercises, stretching maneuvers, hands-on therapy, and equipment to ease patients pain and help them increase their mobility
3. Work cooperatively with physicians and office staff
4.Monitor, document, and report patients progress
Skills and Abilities:
· Responsible, reliable, self-directed and able to work with little supervision.
· Appropriate clinical judgment and communication skills
· Ability to establish and maintain courteous and cooperative (service oriented) working relations with patients, families and co-workers
· Ability to use Electronic Medical Software to record documentation.
· Home Health experience preferred but not required
