Job Overview
The Registered Nurse (RN) provides clinical care, health education, medication coordination, and reentry-related medical support to incarcerated individuals eligible for Apple Health Reentry Initiative benefits. The RN ensures compliance with Washington State Health Care Authority (HCA) guidelines, supports required health screenings, assists with eligibility and release planning, and delivers patient-centered care to promote safe, coordinated reentry into the community.
Responsibilities
1. Medical Intake & Health Screening
- Conduct initial medical intake screenings within required timeframes to determine acute and chronic care needs for individuals entering the facility.
- Perform ongoing assessments, vitals, symptom review, medication reconciliation, and triage of medical conditions.
- Participate in mandatory health screenings as outlined in Reentry Initiative benefit services.
2. Apple Health Reentry Initiative Support
- Assist in verifying Apple Health eligibility, gathering health documentation, and supporting applications for individuals needing coverage reinstatement or enrollment.
- Communicate with Apple Health Managed Care Organizations (MCOs) when needed to coordinate member materials or clinical information.
3. Medication Coordination & Reentry Pharmacy
- Coordinate medication access for short- and long-term stays, including pre-release medications required under Reentry Pharmacy policy.
- Ensure accurate administration, documentation, and education regarding medications.
- Prepare discharge medication packets in accordance with Reentry Pharmacy guidelines.
4. SUD/MAT (Opioid Use Disorder) Support
- Support medication-assisted treatment (e.g., methadone continuation) by coordinating with OTP providers, prescribers, or community programs.
- Ensure proper documentation, tracking, and compliance with federal/state regulations.
5. Care Coordination & Release Planning
- Collaborate with internal multidisciplinary teams, care managers, and external OTP or community providers to ensure continuity of care at release.
- Provide patient education regarding follow-up appointments, medication adherence, chronic condition management, and available community-based supports.
- Assist in gathering medical information needed for discharge planning and reentry.
6. Documentation & Compliance
- Document all assessments, medication administration, interventions, and health encounters in accordance with HCA, facility, and regulatory guidelines.
- Maintain accurate Apple Health–related records for eligibility, benefits, and reentry services.
- Follow all Reentry Initiative guidelines, including staffing structure expectations, privacy requirements, and service delivery standards.
Requirements
- Current Washington State Registered Nurse (RN) license.
- Experience in correctional health, emergency nursing, community health, behavioral health, or similar setting preferred.
- Knowledge of trauma-informed care, chronic disease management, and substance use disorder care.
- Familiarity with Medicaid/Apple Health, MCO processes, and reentry-related services (training available).
- Ability to work collaboratively with interdisciplinary teams, including mental health, medical, pharmacy, and case management staff.
