About the Role:
As a valued member of the Oil City Nursing and Rehab team, we are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our family of caregivers. Located just outside the hustle and bustle of town in a peaceful, comfortable setting, our facility is dedicated to providing personalized customer experience and supportive care to our residents.
Responsibilities:
Provide direct patient care to our residents, including but not limited to vital signs, medication administration, and wound care as ordered by the attending physician
Assist with ADLs (Activities of Daily Living) such as bathing, dressing, and grooming
Develop and implement individualized care plans, in collaboration with the multidisciplinary team
Monitor and document resident progress, reporting any changes or concerns to the nursing staff
Work closely with the LPN/LVN team to ensure consistent care and communication
Participate in quality assurance activities to maintain high standards of care
Requirements:
Current licensure as an LPN/LVN in the state of Pennsylvania
High school diploma or equivalent required
Basic Life Support (BLS) certification required
Ability to work effectively in a fast-paced environment with a focus on patient-centered care
