Job Description:We are seeking a compassionate, knowledgeable, and self-motivated Clinical Liaison with a nursing background and experience in respiratory care to support our skilled nursing facility in Middlefield, Ohio. Our facility includes specialized ventilator units, and this role is essential in helping patients transition smoothly from hospitals to our high-acuity care setting.
The ideal candidate is a go-getter who thrives on building relationships and enjoys being out in the field, connecting with referral sources across the Cleveland, Akron, and Pittsburgh regions.
Key Responsibilities:
- Act as the primary liaison between hospital case managers, physicians, and our facility’s vent units * Evaluate and coordinate patient admissions, particularly those requiring ventilator or respiratory support * Provide education on our respiratory services to hospitals, patients, and families * Ensure seamless transitions of care with a strong clinical understanding of respiratory and complex medical conditions * Build and maintain referral relationships across hospitals, LTACHs, and SNFs * Represent our facility at community events, professional meetings, and outreach opportunities
Qualifications:
- Active RN or LPN license in Ohio * Clinical experience in respiratory, pulmonary, or critical care nursing required * Strong communication, relationship-building, and organizational skills * Self-starter who is comfortable working independently and managing a travel-based role * Valid driver’s license and reliable transportation * Prior experience with vent patients and admissions coordination preferred
