As a Hospice Registered Nurse (RN) Case Manager at Ohioans Home Healthcare & Hospice, you will play a key role in an organization with a reputation for unsurpassed quality care, a close-knit, family-friendly culture, and a supportive management team as we begin to introduce hospice services to our Sandusky area territory.
We will rely on you to assess, plan and implement patient care; educate patients and families on comfort care processes, procedures and medications; and address various patient needs.

Requirements
We are looking for candidates with:
- One year of comfort care experience
- Current RN state licensure
- CPR certification
- Ability to travel for in-home patient visits
- Exceptional time-management skills
- Excellent interpersonal and customer service skills
- Hospice/case management experience preferred
- This position will include an on-call rotation.
