Job Summary
We are seeking compassionate and dedicated Spanish Speaking Home Health Aides to provide exceptional in-home care to clients across various settings. The ideal candidate will assist with daily living activities, monitor patient health, and support overall well-being. Responsibilities
- Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support
- Monitor and observe patients’ conditions, reporting any changes or concerns promptly
- Administer medications as prescribed and document medication administration accurately
- Take and record vital signs including blood pressure, pulse, temperature, and respiration
- Support patients with medical procedures such as catheter care, wound care, or using Hoyer lifts
- Assist with meal preparation and feeding to ensure proper nutrition
- Perform light housekeeping tasks including cleaning, laundry, and maintaining a safe environment
- Document patient care activities accurately using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems in compliance with HIPAA regulations
- Provide emotional support and companionship to enhance patients’ quality of life
- Assist with transportation to medical appointments or community activities when needed
- Follow care plans developed by healthcare professionals and communicate effectively with family members and healthcare teams
Skills
- Proven experience in home health care, assisted living, nursing homes, or long-term care environments
- Knowledge of medical terminology and documentation standards for medical records
- Ability to work with diverse populations including seniors, people with developmental disabilities, dementia or Alzheimer’s disease, hospice patients, and those requiring memory care or hospice care
- Skilled in patient observation and recognizing signs of health deterioration or emergencies
- Proficiency in using EMR/EHR systems for accurate record keeping
- Certification in CPR is required; additional certifications such as medication administration or hospice care are a plus
- Experience working with people with disabilities or special needs preferred
- Strong caregiving skills including assisting with ADLs, medication management, catheter care, and dementia care
- Ability to work independently while following established care plans and protocols
- Compassionate attitude with excellent communication skills to interact effectively with clients, families, and healthcare professionals
