What are the responsibilities and job description for the COMMUNITY LIVING SPECIALIST position at Ohio District 5 Agency On Aging?
Summary
This position is established for the purpose of providing support to functions of the Community Living Division.
Essential Functions
Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.
- Provides assistance to the Community Living Division with delegated tasks as needed.
- Completes components of the HEMEN process as assigned and according to established procedures.
- Completes components of Levy/Title III transportation and Home Delivered Meals process as assigned and according to established procedures.
- Completes components of the Senior Farmer’s Market Nutrition Program (SFMNP) as needed.
- Provides support to new programs/pilot projects as requested by division leadership.
- Supports activities related to agency Wellness Programs as needed.
- Access A&D system to document individual eligibility and services.
- Contacts and networks with physicians, other social service agencies, providers and the Agency’s staff as needed to coordinate service for individuals and obtain signatures as needed.
- Maintains documentation of work activities in A&D, Microsoft Word and client filing according to the Agency’s and ODA specifications and required timeframes.
- Completes Annual Reassessments as part of Eligibility Redetermination for Home Delivered Meal individuals by due dates as needed.
- Completes quarterly contacts with Care Coordination individuals by required time frames and notifies care manager of any concerns the individual’s voice.
- Completes components of the central referral process as assigned and according to established procedures and timeframes.
- Understands the Agency’s Conflict of Interest Policy and complies with the Ohio Aging Network’s requirements for disclosure.
- Participates in training to maintain and improve job skills.
- Assists with and completes special projects and activities as assigned.
- Maintains strict confidentiality of consumer information according to Agency policy.
- Assists in maintaining & upgrading computer applications.
- Strong verbal and written communication skills.
- Good computer skills in Microsoft word, Excel, and Publisher.
- Ability to communicate professionally, respectfully, and effectively.
- Knowledge of and experience in office procedures.
- Ability to work in a fast-paced environment and multi-task.
- Ability to perform assignments with minimal supervision.
- Ability to make sound decisions and demonstrate critical thinking skills.
- Ability to prioritize, organize and maintain accuracy in a flexible job environment.
- Ability to relate well with all individuals, community agencies, and Area Agency staff.
