Position Summary
The Case Manager supports youth and families by coordinating services, developing care and discharge plans, facilitating appointments, and ensuring compliance with OhioMHAS and Medicaid requirements. They serve as a liaison between the youth, family, and service providers while advocating for continuity of care across all program types.
Key Responsibilities
· - Coordinate and monitor the provision of services for assigned youth.
· - Supports the youth's individualised treatment and discharge plans.
· - Facilitate appointments and referrals to medical, educational and social services.
· - Ensure accurate and timely documentation for continuity of care.
· - Communicate with families, legal guardians, and service providers to support care coordination.
· - Participate in treatment team meetings and assist with interdisciplinary planning.
· - Assist youth in accessing benefits, housing, education, and employment resources.
· - Support quality improvement and CARF compliance efforts.
Qualifications
· - Bachelor’s degree in Social Work, Psychology, Human Services, or related field. Or equivalent work experience.
· - Experience working with adolescents in a behavioral health, school or residential setting preferred.
· - Valid Ohio driver's license and ability to transport clients if needed.
Skills & Competencies
· - Strong organizational and coordination skills.
· - Effective communication and documentation.
· - Understanding of Medicaid, and CARF standards.
· - Ability to advocate for youth and families in diverse systems.
Job Types: Part-time, Contract, Internship
Pay: $17.00 - $21.00 per hour
