JOB DESCRIPTION – Ogemaw Commission on Aging
Supervisor: In Home Service Coordinator
Special considerations:
- Requires use of personal vehicle and ongoing proof of current insurance
- Requires minimum 2 weeks’ notice of resignation (see Handbook for details)
QUALIFICATIONS:
- Must be mature individual able to relate to senior population.
- Must be able to maintain confidentiality regarding participant information.
- Organized, able to keep accurate records.
- Computer literate – competent in Microsoft Office Excel and Word
- Excellent communication skills - interviewing, critical listening
- Ability to work independently and meet deadlines.
- Reliable transportation, current driver’s license, and proof of insurance.
DUTIES:
- Complete all reassessments for In Home Service participants on schedule.
- Report changes in participant status, unmet needs, requests for change of service, etc. to the In Home Service Coordinator
- Keep IHS Coordinator apprised of any current or potential problems regarding participants or staff
- Assist in keeping participant case records and master list up to date.
- Make copies of assessment forms and provider paperwork
- Complete assessments if requested by the In Home Service Coordinator.
- Conduct observations of In Home staff if requested by the In Home Service Coordinator.
- Wear COA identification at all times when representing OCOA.
- Attend all necessary meetings and trainings as scheduled.
- Abide by all State, Federal, AASA, and COA standards.
- Assume any additional duties as assigned by the In Home Service Coordinator.
PHYSICAL DEMANDS:
· Work 5 to 6 hours/day, sitting/standing/moving
· Ability to grip/grasp - throughout day
· Vision sufficient to read paperwork
· Typing, writing, reading, comprehending written instructions
· Talking, in person and/or on phone
· Hearing/listening sufficient to communicate in person or on phone
· Viewing data on computer screen
· Driving
· Lifting 0 to 20lbs
WORKING ENVIRONMENT
· Indoor office, temperature controlled
· Indoor - in participant’s home
· Driving personal vehicle/various businesses, occasional travel
· Variety of private homes-walkways, stairways, ramps, etc.
· Some exposure to pets/smoking
· Travel on a variety of road types-paved, dirt, occasionally private trails-some in poor condition
Job Type: Part-time
Pay: $13.75 - $14.75 per hour
