Key ResponsibilitiesConduct community outreach to build trust, identify needs, and connect individuals to relevant programs and resources

Assess basic service needs and make appropriate referrals to internal programs and community partners
Maintain accurate records of outreach activity, referrals, and follow-up outcomes in accordance with program requirements
Support outreach planning and team coordination to ensure coverage of assigned priorities and targets
Demonstrated experience in outreach, customer/client-facing roles, community engagement, or related support work
Strong interpersonal skills with the ability to communicate clearly and professionally with diverse individuals
