Role and Responsibilities

Primary responsibility is to ensure that every referral is processed and reviewed in a timely manner and that every admission is accomplished by providing a smooth transition from the hospital, community or other setting. Serve as the communication conduit between and among departments on every referral and every admission. Assist in positioning the skilled nursing location as a leader in short-term rehabilitation and long-term care with the goal of contributing to build and grow census and quality resident mix.

Plans and develops marketing material and events that meet our care objectives.

Essential Duties and Responsibilities:

1. Must have a working knowledge of admission and marketing practices used in enhancing census.

2. Remains in constant communication with nursing staff about admissions.

3. Complies with PDPM and completed necessary paperwork and distribute to all necessary departments

4. Has knowledge of State and Federal guidelines and regulation affecting Nursing Homes and Assisted Livings

5. Provide an updated roster at least 3 times a week

6. Coordinate with Unit Coordinator and Clinical Managers to ensure all standard supplies needed to care for resident are in place prior to admission (O2 signs, isolation signs, equipment, etc.).

7. If a referral is denied, ensure the administrator is notified of the denial

8. Provide clinical team with information concerning anticipated admissions, prior to admission and re-admissions.

9. Assess potential admissions at home, assisted living, and/or hospital as needed

10. Attends meetings with hospital and community partners

11. Attend daily stand up with information about new referrals and pending referrals

12. The ability to organize and implement a marketing program that encompasses all three care levels.

13. Attain and/or maintain the targeted occupancy levels of all 3 buildings.

14. Able to assess a residents’ appropriateness or inappropriateness for admission based on information gathered at the referral source.

15. Ensure that gift bags are provided to each unit coordinator, so that they can distribute to each new admission

16. Communicate to Clinical Manager and Unit Coordinator accepted admissions and approximate arrival times prior to admission

17. Make on site visits with potential residents to gather pertinent admission information as needed

18. Have a working knowledge of Medicare and Medicaid reimbursement

19. Be, or be willing to become, familiar with multiple insurance providers for reimbursement purposes.

20. Function as a referral source with a working knowledge of our community resources for residents we are unable to service.

21. Be familiar with all of the resources in our community that offer senior services.

22. Marketing of our skilled facility and our assisted living facilities in the community.

23. Organize and act as a facility in-take contact for all interested individuals, referrals, and admissions, for all buildings

24. Has the ability to offer a caring, concerned atmosphere for residents and families that are seeking admission.

25. Offer and conduct tours of all 3 facilities.

26. Participates as a member of the QAPI committee and provide a summary report of all admissions, discharges, referrals, inquiries, and discharges. Provides monthly reports of the same to the Nursing Home Administrator and the CEO

27. Maintain the accurate daily census for all 3 facilities and report on them during stand up meetings

28. Complete and submit Medicaid Applications and complete renewals as needed.

29. Maintains a community resource file.

30. Keeps current on market trends and acts as a liaison between the facilities and the community

31. Participates in in-service training as appropriate.

32. Has strong communication skills, interpersonal skills, and ethics.

33. Is well groomed and adheres to the dress code of business casual

34. Complies with Facility Privacy Practices as governed by HIPPA

35. Works closely with the Director of Nurses, the Nurse Manager, and the Social Worker to determine admission appropriateness.

36. Makes daily contacts, as necessary, to the area hospitals to secure admissions and maintain census.

37. Establish a relationship with area Physicians and their Nursing staff.

38. Maintains good records of inquiries, admissions, and refusals.

39. Reviews Admission Contracts with the resident and appropriate family members or AHPOA.

40. Other duties as delegated by the CEO and/or the Administrator.

41. Puts admit packets together and provide gift bags for all new admissions

42. Maintains Admissions phone

43. Able to navigate through MATRIX for admissions and discharges

44. Completes Precertification’s for insurances prior to a resident being admitted

INTERPERSONAL SKILLS:

· Experience in health care and/or human service, marketing, census development and the admission process.

· Strong verbal and written communication skills.

· Ability to work well in a team setting.

· Positive and enthusiastic approach to tasks.

· Participation in professional organizations and workshops.

· Appropriate, neat and professional grooming, manners and conduct.

· Proficient in Microsoft Word/Excel/Publisher and email.
Qualifications and Education Requirements

  • Has a bachelor’s degree in communications, marketing or a related field or related experience.
  • Has at least one year of experience directing admissions in a long-term care setting.
  • Is creative with the ability to think outside of the box.
  • Some knowledge of HSS 132, HSS 89, and HSS 83 and skilled Nursing Facilities admission limitations or the ability to learn
  • Has basic computer skills.

Job Type: Full-time
