Position Summary
Our client is seeking a compassionate, mission-driven Behavioral Health Provider to join their integrated care team serving patients across the lifespan. This role is ideal for a licensed or license-eligible clinician with a strong foundation in training, experience working with trauma, and the ability to support children, adolescents, and adults in a diverse, community-based setting.
The Behavioral Health Provider will offer comprehensive behavioral health services—including assessments, brief interventions, psychoeducation, and ongoing therapy—in the clinic. This individual will collaborate closely with medical, dental, psychiatric, and educational professionals to deliver whole-person care and promote overall well-being. The successful candidate will demonstrate the ability to engage patients and families with empathy, apply evidence-based interventions, and maintain a flexible, team-oriented approach within a fast-paced, integrated care model.

Essential Functions:
- Maintain a visible presence during clinic hours and collaborate with providers through same-day consultations or feedback.
- Offer “curbside” consultations and schedule initial patient assessments in response to referrals.
- Serve as a clinical resource for colleagues and the broader community.
- Promote integrated care through active collaboration with medical and clinical staff.
- Conduct patient outreach and follow-up, including support for medication response and case management.
- Deliver brief interventions and psychoeducation for patients in individual settings.
- Participate in chronic disease management programs and risk assessments.
- Maintain and review educational materials to ensure cultural and literacy appropriateness.
- Triage high-risk behavioral cases and refer to appropriate resources.
- Document all patient interactions in the electronic health record.
- Participate in program evaluation, quality initiatives, and team development.
- Support aftercare planning following hospitalization or high-level care.

Qualifications:
Must Haves:
- Master’s degree or higher in a recognized mental health field (e.g., Counseling, Social Work, Marriage and Family Therapy). Active or license-eligible in Colorado (LPC, LCSW, LMFT, LSW, or equivalent).
- Registered with the Department of Regulatory Agencies (DORA).
- Demonstrated experience in assessing, diagnosing, and treating mental health conditions across the lifespan.
- Foundational training in trauma-informed care and evidence-based psychotherapy. Understanding of clinical ethics, cultural diversity, treatment planning, and integrated care practices.
- Proficient in EHRs and Microsoft Office applications.
- Strong interpersonal skills with the ability to resolve conflict, collaborate, and work empathetically with patients and team members.
Nice to Haves:
- Two years of relevant clinical experience is preferred.
- Training in evidence-based therapies such as , DBT, and CBT.
- Familiarity with psychopharmacology and behavioral medicine.
- Experience in couples and family therapy.
- language proficiency is a plus.

Pay: $72,000.00 - $85,000.00 per year
