Apollo Home Healthcare is hiring a full-time or part-time/per diem Physical Therapist (PT) to join our Home Health team, serving patients in Northern California's Bay Area. In this role, you will provide high-quality physical therapy services to patients in the comfort of their own homes, helping them regain independence and improve their daily functioning. You will oversee a supporting Physical Therapy Assistant and a team of dedicated healthcare professionals who are committed to delivering exceptional patient-centered care.

- Provide in-home physical therapy evaluations and treatments
- Assess patients' strength, mobility, balance, and ability to function safely at home
- Create and update individualized plans of care that support independence and recovery
- Help patients regain mobility and confidence in their home environment
- Educate patients and caregivers on exercises, safety, and fall prevention
- Document visits and evaluations in WellSky/Kinnser in a timely manner
- Collaborate with nurses, therapists, and the broader care team to support patient outcomes
- Supervise Physical Therapy Assistants (PTAs) and support their clinical development
- Participate in care planning, case conferences, and discharge planning
- Graduate of an accredited Physical Therapy program (Bachelor's, Master's, or DPT)
- Active California Physical Therapist license
- CPR/BLS certification
- Strong communication and organizational skills
- Reliable transportation and willingness to travel to patients' homes
- Comfortable working independently in a home health setting
- Home health experience preferred, but not required
- $40-170/hour
- Mileage reimbursement
