Position Overview
The HOPE Coordinator plays a vital role in transforming lives by connecting clients with the resources and services they need to thrive. This full-time position focuses on community involvement, resource and service linkage for clients served at OATH Recovery, Inc. The HOPE Coordinator is responsible for linking clients with housing support services, governmental benefits, and medical, mental health, and substance use disorder treatment and peer support services. The HOPE Coordinator acts as a liaison between clients and other service providers. This is a grant funded position that is time-limited. The HOPE Coordinator is responsible for collaboration and coordination of mental health and substance abuse services, acting as the liaison between clients, OATH, and Unison Behavioral Health to strengthen care coordination and meet grant deliverables.

Key Responsibilities
· Coordinate services and complete associated screenings for OATH Recovery, Inc. clients, linking adults, children, adolescents, and their families to individual and group counseling, recovery, and support services via telehealth or in-person appointments.
· Assist OATH clients in obtaining housing.
· Establish and maintain a referral network for OATH clients and collect data regarding referrals.
· Develop collaborative relationships with schools, community centers, and other community resources to facilitate referrals for social and medical services as indicated by client need.
· Connect clients with employment opportunities, healthcare, SSI/SSDI, TANF, and other benefits to help restore their health and well-being.
· Coordinate transportation and other support services to ensure successful treatment and recovery outcomes.
· Help eligible clients enroll in medical insurance, Medicaid, and Georgia's Peach Care for Kids Program.
· Assist clients and families with post-enrollment activities, including benefit re-authorization and appeals of coverage decisions.
· Serve as an active, empathetic listener — identifying barriers such as stigma or transportation challenges — and develop individualized solutions.
· Provide follow-up engagement to ensure successful treatment and resource linkage.
· Attend regularly scheduled IMPACT Network meetings, OATH Board of Directors meetings, Recovery Center Organization implementation meetings, and TRUST Partnership Coalition meetings.
· Maintain records of client services, prepare reports for the OATH Board of Directors, and collect and report data on care coordination services.
· Maintain staff schedules, timecards, leave records, payroll registers, and records of office income and expenses.
· Implement certified peer specialist support services at OATH and All Recovery group meetings.
· Supervise all office employees and volunteers.
Qualifications
Required:
· High school diploma and three (3) years of social service delivery to underserved populations; OR an associate degree in a social service delivery field and one (1) year of experience; OR a bachelor's degree in psychology or a related field from an accredited college or university and one (1) year of experience working in mental health, substance use disorder services, or a related field.
· Case management experience is a plus.
Preferred:
· Certified Peer Specialist (CPS) or Certified Addiction Recovery Empowerment Specialist (CARES) eligibility.
Skills & Knowledge
· Knowledge of diagnostic criteria and how client advocacy systems operate.
· Familiarity with individual and group counseling techniques, various therapeutic models, and crisis intervention.
· Excellent written and verbal communication skills with strong interpersonal abilities.
· Strong organizational skills with the ability to plan, prioritize, and adapt in a dynamic environment.
· Ability to build and sustain relationships with clients, staff, the Board of Directors, and community partners.
· Proficiency in Google Workspace, Microsoft Office Suite, Homeless Management Information System (HMIS), Publisher, and Adobe Suite.

Reports To: Board of Directors and Healthcare Navigator
Supervises: OATH Recovery, Inc. staff and volunteers
