Oasis Therapy Services is seeking a Team Lead Physical Therapist Assistant to provide one-on-one rehabilitation services and assist with therapy operations in independent living, assisted living, and memory care settings in Litchfield, Minnesota.

Responsibilities
- Provide physical therapy interventions directed by and under the supervision of a licensed Physical Therapist.
- Carry out treatment plans focused on strength, balance, gait, mobility, safety, and independence.
- Monitor residents' progress and report changes or concerns to the supervising Physical Therapist.
- Participate in care conferences within the PTA's scope of practice.
- Serve as a liaison among residents, facility staff, and the therapy team.
- Assist the Director of Rehabilitation with assigned administrative and non-clinical managerial tasks.
- Complete accurate and timely Net Health documentation.
Qualifications
- Active and current Minnesota Physical Therapist Assistant license.
- Graduate of a CAPTE-accredited Physical Therapist Assistant program.
- Current CPR certification.
- Strong leadership, communication, organization, and documentation skills.
- Geriatric, independent living, assisted living, memory care, or rehabilitation experience is preferred.
