Role Description
This is a part-time on-site role for a Home Health Care Coordinator, based in Miami, FL. The Home Health Care Coordinator will oversee care plans, coordinate services for patients, and communicate effectively with healthcare professionals, patients, and their families to ensure high-quality care. Responsibilities also include scheduling, maintaining accurate records, and addressing any patient concerns while adhering to regulatory requirements and company standards.
Qualifications
- Strong organizational, scheduling, and time management skills for managing patient care plans and services
- Exceptional communication and interpersonal skills to liaise with healthcare providers, patients, and families effectively
- Knowledge of healthcare regulations, patient confidentiality, and documentation best practices
- Experience in home health care coordination, healthcare administration, or related fields
- Proficiency in computer systems, including scheduling software and electronic health records
- Problem-solving skills and the ability to adapt to rapidly changing situations
- Bachelor’s degree in healthcare administration, nursing, or a related field; additional certifications in care coordination are a plus
- Bilingual proficiency (Spanish/English) is beneficial for this region
