Job Description

We are currently hiring for two peer support / community health workers

The Oasis Center is a nonprofit family medical clinic and we are looking for qualified and compassionate individuals to join our team!

This position supports patients experiencing substance use disorders who are pregnant or have young children. This position will promote recovery and family stabilization by supporting patients and connecting with community resources. This position is an essential part of the team.

A successful candidate who is not yet certified as Community Health Workers (CHW) through the Oregon Traditional Health Worker registry will be able to complete this training after hire.

Job Responsibilities:

  • Work in collaboration with the Oasis Care Team and Medical providers to support patients experiencing substance use disorders to engage in care, treatment, and to navigate resources
  • Manage a caseload of patients
  • Document in electronic health record
  • Role model recovery, parenthood, and self-efficacy
  • Provide patient transportation within community to support health and recovery goals
  • Maintains a high level of confidentiality

Requirements:

  • High School diploma, or equivalent
  • Previous experience in peer support or healthcare preferred
  • Current and valid Oregon Driver’s License and able to be insured
  • Peer Support Certification preferred
  • Community Health Worker certification required; able to obtain after hire.
