Job Description:
The Clinical Case Manager is responsible for ensuring the provision of advocacy services to consumers residing in the PRTF serving as a community liaison for the PRTF and performing administrative advocacy responsibilities for the facility. Clinical Case Manager provides consumers ongoing support and expertise through comprehensive assessment, planning, implementation and overall evaluation of individual treatment needs. The CCM is accountable for the admission, treatment coordination and discharge planning of assigned consumers. Working in cooperation with other professional staff and exhibits cultural sensitivity toward consumers, stakeholders and co-workers from various cultural, socioeconomic, ethnic, religious and racial backgrounds.
Responsibilities:
- To provide direct advocacy services to the consumers of the facility.
- Serves as liaison/contact person for consumer families and guardians.
- Provides initial development and ongoing assessment of the consumer’s Person Centered Plan and treatment goals.
- Provides initial development and ongoing assessment of the consumer’s Behavioral Health Treatment Plan.
- Provides initial development and ongoing assessment of the consumer’s Transition Plan.
- Facilitates and documents the consumer’s monthly Child and Family Treatment Meeting.
- Serves as liaison/contact person and communicates any change in consumer status to the Treatment Team, RN/LPN, Client Rights Committee and consumer’s guardian and other stakeholders.
- Provides case management functions of arranging, linking and/or integrating of multiple services as well as the assessment and reassessment of the consumer’s needs for services.
- Facilitates consumer step-down and discharge. Prepares Discharge Summary in accordance with OAKWOOD TREATMENT CENTER policy.
- Facilitates the consumer, Care Coordinator, and guardian in completing a Discharge Satisfaction Survey and forwards information the Quality Assurance Coordinator.
- Facilitates external stakeholders in completing the Internal/external Stakeholder Satisfaction Survey and forwards information to the Information Measurement Manager.
- Provides follow-up contact with consumer, guardian within thirty days of discharge to determine post-discharge status. Contact will be documented and information forwarded to Quality Assurance Coordinator.
- Provides Administrator On-Call duties.
- Participates in Psychiatric Clinic.
- Ensures licensure guidelines and regulations in relating to advocacy are implemented.
- Relays policies, programs, services and relative information to the agency staff, consumers, guardians, consumer rights committee members and surveyors.
- Provide and coordinate investigative services in the area of consumer abuse, neglect, and exploitation inquiries and ensure compliance with reporting mechanisms.
- Provides input to the Director and programmatic staff on issues pertaining to consumer advocacy, consumer rights issues, and community resources.
- Assists in coordinating and serving as a member of the screening and admissions committee.
- Liaison with outside stakeholders/agencies to maintain referral base for potential PRTF placement.
- Maintains documentation consistent with OAKWOOD TREATMENT CENTER policies and protocols.
- Participates in workshops, seminars, etc. pertinent to the position as approved by the Director.
- Serves as an ex officio member of the Consumer Rights Committee.
- Manages the Judicial Review process.
- Coordinates discharge activities and placements.
- Complies with all OAKWOOD TREATMENT CENTER policies and procedures.
- Performs other duties as assigned to include, but not limited to:
- Assisting as directed in emergencies, crises and disasters, beyond regular duties;
- Travel as directed by supervisory staff on behalf of OAKWOOD TREATMENT CENTER;
- Assisting as directed with planned or unplanned moves of offices, facilities and/or consumers.
- Attendance and/or participation in OAKWOOD TREATMENT CENTER planned or sponsored events as assigned;
- Attendance and participation in training, staff development or events related to OAKWOOD TREATMENT CENTER business as assigned.
Qualifications:
· Masters Degree in a human services field and 2 years post graduate, work experience in the area of child mental health.
· Bachelors Degree in a human services field and 3 years post graduate, work experience with child mental health.
· RN with two years of full-time, post licensure accumulated MH/DD/SA experience with population served
Pay: $20.00 - $25.00 per hour
