Position: Part-Time Occupational Therapist (OT)

- Evaluate referred residents to determine functional level and rehabilitation potential
- Provide direct care and develop individualized treatment plans for ADL (Activities of Daily Living) improvement
- Establish treatment goals based on resident needs, limitations, and physician input
- Implement therapeutic interventions to enhance strength, coordination, mobility, cognition, and self-care
- Educate residents and families on carryover programs and adaptive strategies
- Accurately complete required documentation including evaluations, progress notes, and discharge summaries
- Collaborate with the interdisciplinary team to support holistic care
- Follow all applicable federal/state regulations, including HIPAA and Residents' Rights
- Perform other duties as needed, as directed by the Director of Rehabilitation
- Graduate of an accredited Occupational Therapy program
- Valid Occupational Therapy license in the state of practice
- Experience in long-term care or geriatrics preferred, but not required
- Strong interpersonal and organizational skills
- Compassionate, flexible, and committed to resident-centered care
