Schedule: Monday-Friday 8:30am-5pm (Some evenings and weekends as needed)
Salary: $65,000/year

Responsibilities
- Engage and assess consumer strengths and needs
- Provide written clinical eligibility assessments and level-of-care recommendations
- Determine clinical eligibility and appropriateness for diversion.
- Refer participants to appropriate treatment providers and services.
- Conduct initial and on-going assessments and adjust care plans as clinically indicated.
- Coordinate with treatment providers regarding participant progress
- Conduct in-person evaluations at: Burlington County Jail and Atlantic County Jail Burlington County female inmates housed there), as needed
- Assist the Case Manager with navigating community support to resources available across the community (as needed)
- Work across multiple counties based on program need
- Attend monthly MHDP, Oaks, and treatment team meetings and court hearings, as needed.
- Collaborating with the Clinical Team, Judicial staff, and other stakeholders
- Assist in the development of the individualized service plan
- Conduct comprehensive clinical interviews of Mental Health Diversion Program applicants
- Review relevant medical, psychiatric, psychological, and correctional records
- Collaboratively develop person-centered treatment plans to address individuals’ goals
- Engage in crisis intervention when necessary
- Maintain required level of service
- Provide supportive telephone contact, as needed
- Maintain clinical records that meet all applicable agency policies for timeliness, regulatory and professional standards, and thoroughly document qualitative aspects of care
- Travel throughout the state of New Jersey, and out of state travel in the tri-state area as needed
- Cooperate with all investigations, internal and externally
- Provide 24 hours/7 days a week on call crisis intervention on a rotating basis, including evenings and weekends, as needed

- Master’s degree in a recognized mental health discipline; licensure as an LSW/LCSW, LPC, LMFT; with a minimum of 3 years’ post-licensure experience required;
- Valid driver’s license in good standing.
