This full time position will office out of Brainerd, MN with possible work and day travel primarily in Crow Wing, Aitkin, Morrison or Itasca counties and the surrounding area.
As a Designated Coordinator, you will:
- Perform highly responsible duties related to the development, implementation and coordination of Individual Service Plans for persons served with developmental disabilities or mental health needs
- Maintain, review and evaluate pertinent program data related to each individual’s growth and development
- Ensure that services are being correctly implemented
- Provide training to direct care staff as necessary

Oakridge Homes is currently looking for a Designated Coordinator who:
- Has a background in human services
- Has excellent verbal/written communication skills
- Has a valid MN driver’s license and clean driving record
- Is able to pass a DHS background study

Meets the qualifications in education and work experience by providing direct care services to persons with disabilities or persons aged 65 or older as follows:
- baccalaureate degree in a human services related field, and 1 year of full-time work
- associate degree in a human services related field, and 2 years of full-time work
- other education and experience may qualify and be considered
