What are the responsibilities and job description for the Medical Assistant - Chronic Care Management position at Oakdale Community Hospital?
POSITION SUMMARY: Coordinates care for patients with multiple chronic conditions. Conducts monthly check-ins, oversees medication management, and connects patients with community resources to improved health outcomes and prevent hospitalizations.
POSITION REQUIREMENTS:
Experience: One year work experience preferred, in a Medical office setting desired. Certified Medical Assistant strongly preferred.
Knowledge of medical terminology desirable.
Word processing and computer experience preferred.
Education: High School Diploma or equivalent preferred.
Certification: BLS/CPR
