The Social Worker job responsibilities include performing psychosocial assessments, developing and implementing individualized care plans, providing counseling to residents and families, and advocates for residents' rights and needs. This role also includes facilitating communication between residents, families, and staff, coordinating discharge planning, and ensuring compliance with regulations
Key Responsibilities:
- Assessing needs, plan and manage all aspects of our comprehensive activities program, working with department resources, team members, and volunteers
- Getting to know each resident individually to better understand needs and varying interests and coordinate activities accordingly
- Completing screens and orientating residents and families to Oak Park Place
- Helping residents and families adjust to the center
- Assisting families and residents with financial questions and referrals
Social Worker Qualifications:
- Associate degree preferred
- Previous experience in a supervisory role
- Previous experience working with seniors is preferred
- Strong organizational and planning skills required
- Ability to work independently is a must
