Position Summary: This employee has the responsibility for coordinating the total plan of care for the residents. The individual may have had advanced training in this area. The work requires administrative ability, knowledge of training techniques, ability to adapt techniques and exercise initiative and good judgment. The work is carried out according to standards of professional nursing, facility policies, rules and regulations, and State and Federal standards.Responsibilities:Collaborates with Facility Staff daily to exchange information regarding resident care, condition and matters affecting resident well-being.Coordinates resident care meetings/care plan conferences involving residents, staff and families within the fourteenth (14) and twenty-first (21) day of admission; then minimal every ninety (90) days and as needed.Completes the Comprehensive Assessments (MDS) within fourteen (14) days of admission, after a significant change and quarterly.Completes the Comprehensive Assessment (MDS) annually on the year’s anniversary.Evaluates admissions within the three (3) working days of admission.Initiates a preliminary plan of care within twenty-four (24) hours of admission and a comprehensive within seven (7) days of completion of comprehensive assessments.Coordinates the completion and accuracy of the Minimal Data Set (MDS) and plans of care according to State and Federal standards.Attends Morning Meetings to gather information and update care plans as needed.Transmits Minimal Data Set (MDS) information according to State and Federal standards.Provides in-service training as necessary.Coordinates and conducts weekly CMI and Medicare meetings.Notifies Resident and/or Family of care plan conferences at least seven (7) days in advance and maintain record of the notices.Participates in the weekend facility Management supervisory rotation.Assists in development and implementation of the facility’s Quality Assurance Performance Improvement Program (QAPI) monthly.Performs other duties as requested within scope of nursing.Our Benefits Include:Paid vacation, holidays, and sick days, subject to our policy.Medical, dental, vision, life, and short-term disability insurance.401(k) plan with employer matching contribution.Paid vacation, holidays, and sick days, subject to our policy.IND123
