Salary Description $270,000-$350,000

- Provide full-scope primary care including preventive, acute, and chronic disease management
- Lead and collaborate within a multidisciplinary care team
- Perform exams, minor procedures, and prescribe per NYS guidelines
- Coordinate care across services, including referrals, labs, and follow-up
- Engage in quality improvement and population health initiatives
- Participate in community outreach and program development
- MD or DO from an accredited medical school
- Active New York State license and DEA
- Board Certified or Board Eligible (within 18 months)
