Duties Description

Under the supervision of the Unit Supervisor, this position provides essential services for the Regional Field Office and Coincides with the expectations of the core mission of OPWDD to provide necessary services to individuals with Disabilities. As a Front Door Facilitator/Children’s Waiver Liaison incumbent will perform administrative duties related to the operation of the Front Door Facilitator/Children’s Waiver Unit. Keeping apprised of new initiatives as well as keeping current with changes in existing programs is imperative to the liaison.

Travel within the Region may be required, as well as intermittent travel to other areas of the state

  • Contact families coming to the Front Door to provide information and guidance on the eligibility and service access (ESAL)process.
  • Request TABS number and provide to families and Care Managers/Service Access agencies
  • Complete Developmental Disabilities Profile 2 assessments
  • Open EAA tool and enter identifying information.
  • Track activities to ensure timely progression of individuals through the Front Door/ESAL process
  • Keep families apprised of progress through the Front Door/ESAL process.
  • Inform and assist families of the process of transitioning from Children’s Waiver to the HCBS Waiver
  • Direct families to the Coordinated Assessment Evaluation process
  • Review Home and Community Based Waiver (HCBS) submissions
  • Send missing or more information letters when necessary
  • Once all information is gathered, cases are submitted to the Quality Review Team (QRT) for service authorization review.
  • Process Notice of Decisions.
  • Close EAA tool and engage in lost contact protocol, when necessary.
  • Troubleshoot problems/issues with assigned cases in the ESAL process
  • Participate in weekly Front Door team meetings and trainings, as scheduled.
  • Participate in OPWDD functions and community events during evening hours and on weekends occasionally
  • Complete mandatory trainings.
  • Participate in special projects as assigned by supervisor

Serves as a member of a multidisciplinary treatment team; participates in team meetings, presents clinical information and observations, and provides consultation and assistance to staff in other disciplines; and coordinates care and treatment planning for individuals.

  • Provides a variety of social work and treatment services to individuals and families that include life skills, vocational and career training, family support, and community integration that assists individuals in achieving maximum participation in work and social activities.
  • Assesses and evaluates individual and family needs, strengths, and limitations; and records significant psychosocial events and/or changes in clinical conditions.
  • Provides individual, group, and family therapy; and crisis intervention, stabilization, and follow-up services to individuals who may be a danger to themselves or others.
  • Advocates for individuals’ rights and access to benefits and services to meet their personal and social needs.
  • Helps individuals identify goals and solutions to a variety of short-term mental, emotional, behavioral, and environmental conditions and uses therapy to address illnesses and disorders, including mental illnesses and emotional disturbances; marital and family difficulties; adjustment problems related to acute and chronic illnesses; addiction, substance use, behavioral, and learning disorders; and community problems and social issues.
  • Educates individuals and families about the nature of illnesses and disorders; and provides individuals with vocational, educational, and personal interest opportunities and resources.
  • Establishes and maintains professional relationships with community providers to ensure continuity of care; assists individuals with the development and maintenance of natural supports; and links individuals and families to existing services in the community to ensure continuity of care.
  • Develops effective treatment plans in conjunction with individuals and treatment teams; monitors and evaluates individuals’ progress; and modifies treatment plans or terminates services when appropriate.
  • Prepares and maintains documentation and records, including progress notes, observations, recommendations, and discharge plans.
  • May supervise and train social work interns, and paraprofessional social work and/or direct care staff. Under the supervision of a licensed clinical social worker, licensed psychologist, or psychiatrist, may provide clinical services to individuals, such as the diagnosis of mental, emotional, behavioral, developmental, and addictive disorders; development of assessment-based treatment plans; and provision of psychotherapy.

Minimum Qualifications

Open Competitive: license and current registration as a Licensed Master Social Worker in New York State.

Transfer:

Candidates must have one year of permanent competitive or 55-b/c status in the posted title and be eligible for lateral transfer; OR be eligible to transfer via Section 70.1, 70.4, or 52.6 of Civil Service Law. The transfer cannot be a second consecutive transfer resulting in an advancement of more than two salary grades.

Additional Comments

This title is part of the New York Hiring for Emergency Limited Placement Statewide Program (NY HELPS). HELPS Program titles may be filled via a non-competitive appointment. This means that you do not need to take an exam to qualify, but you do need to meet the minimum qualifications of the title.

At a future date (within one year of permanent appointment), employees hired under NY HELPS are expected to have their permanent non-competitive employment status converted to permanent competitive status. You will not have to take an exam to gain permanent competitive status.

  • Please note that starting salary for this title reflects the downstate adjustment. Posted Hiring Salary includes the sum of the statutory hiring rate for PS&T Grade 20 ($74,193) and Downstate Adjustment ($4,000). Downstate Adjustment is also added to the job rate ($94,121).
  • If you are currently a New York State employee serving in a permanent competitive qualifying title and eligible for a transfer via Section 70.1 of the Civil Service Law, you may be appointed to this title as a permanent competitive employee.
  • The assignment of PS&T employees will be made in accordance with any agreed upon Memorandum of Understanding between PEF and the local DDSO regarding the posting of job vacancies.

  • All OPWDD employees must be eligible and maintain eligibility for full and unconditional participation in the Medicare and Medicaid programs. Continued employment will depend on maintaining eligibility.

The PSLF Program forgives the remaining balance on Direct Loans after 120 qualifying monthly payments have been made under a qualifying repayment plan while working full-time for a qualifying employer.
