Duties Description

POSITION DESCRIPTION:

Under the supervision of the Developmental Disabilities Program Specialist 2, the Licensed Master Social Worker (functional title: Residential School Transition Coordinator (RSTC)) plays an integral role in facilitating planning for youth aging out of residential schools. The RSTC is responsible for coordinating the transition to adult OPWDD services for students aging out of residential school settings. The RSTC will be part of team that is expected to assist with this planning for students from across the 18 counties within the Taconic and Hudson Valley Districts of Region 3. Across the 18 counties within Region 3, over 200 students in residential schools . This number is expected to grow as additional students are found to be eligible for OPWDD services.

POSITION RESPONSIBILITIES:

Duties include but are not limited to:

  • Will have continuous interface with residential schools, families/guardians, the individual, Central Office, and provider agencies around the supports needed for transition from the residential school.
  • Will act as Specialist Liaison while transitioning students from residential schools to adult services.
  • Assists with person centered planning and ensuring connections to residential or in-home supports as well as ensuring identification of day services.
  • Responsible for monitoring and upkeep of the residential age out database.
  • Participate in quarterly meetings for individual’s residing in residential schools to ensure awareness of current needs.
  • Visit individuals at their residential schools to be familiar with theirs needs related to planning.
  • May be needed to participate in Fair Hearings.
  • Interface with the Certified Housing Team
  • Involved in efforts to identify providers to plan and develop services including but not limited to, presentations at the Residential Opportunities Committee and involvement in the Letter of Interest (LOI) process.
  • Ensure the required Preliminary Adult Service Plans are submitted timely and reviewed to ensure they meet the individual’s needs.
  • Assist with completing Developmental Disabilities Profile 2 (DDP2) Assessments
  • Assist with reviewing Higher Needs Funding applications.
  • Respond to requests for information from Leadership within time frames given.
  • Complete other duties as assigned.

Minimum Qualifications

Open Competitive: license and current registration as a Licensed Master Social Worker in New York State.

Transfer:

Candidates must have one year of permanent competitive or 55-b/c status in the posted title and be eligible for lateral transfer; OR be eligible to transfer via Section 70.1, 70.4, or 52.6 of Civil Service Law. The transfer cannot be a second consecutive transfer resulting in an advancement of more than two salary grades.

Additional Comments

This title is part of the New York Hiring for Emergency Limited Placement Statewide Program (NY HELPS). HELPS Program titles may be filled via a non-competitive appointment. This means that you do not need to take an exam to qualify, but you do need to meet the minimum qualifications of the title.

At a future date (within one year of permanent appointment), employees hired under NY HELPS are expected to have their permanent non-competitive employment status converted to permanent competitive status. You will not have to take an exam to gain permanent competitive status.

  • Please note that starting salary for this title reflects the Mid-Hudson adjustment. Posted Hiring Salary includes the sum of the statutory hiring rate for PS&T Grade 20 ($74,193) and Mid-Hudson adjustment ($2,000). Mid-Hudson adjustment is also added to the job rate ($94,121).
  • If you are currently a New York State employee serving in a permanent competitive qualifying title and eligible for a transfer via Section 70.1 of the Civil Service Law, you may be appointed to this title as a permanent competitive employee.
  • The assignment of PS&T employees will be made in accordance with any agreed upon Memorandum of Understanding between PEF and the local DDSO regarding the posting of job vacancies.

  • All OPWDD employees must be eligible and maintain eligibility for full and unconditional participation in the Medicare and Medicaid programs. Continued employment will depend on maintaining eligibility.

The PSLF Program forgives the remaining balance on Direct Loans after 120 qualifying monthly payments have been made under a qualifying repayment plan while working full-time for a qualifying employer. ° Thirteen (13) paid holidays each year.

° Up to three (3) days professional leave each year to participate in professional development events.
