Location Bronxville, New York Shift: Day Flex (United States of America) Description: Make It Possible: Care Coordinator - RN - Social Services - PD - Days At NewYork Presbyterian Westchester Hospital (located in Bronxville, NY), talented RN Care Coordinators lead, learn and educate in a department like no other. We partner across the full spectrum of key areas, providing life-changing research, education and practice. You can help make it possible. As a Care Coordinator, you'll be responsible for providing intensive care management for adult patients with complex medical needs. You will serve as a strong and knowledgeable liaison between the interdisciplinary team of social workers, Physicians, Physical Therapists, Occupational Therapists, Cardiologists, Intensivists – all in support of patients and their families. Direct all aspects of inpatient care, develop individualized care plans, ensuring optimal length of stay. Help prevent hospital readmission, anticipate outpatient service needs, setting appointments and consultations prior to discharge. This is a per diem, day-shift role covering weekend hours from 8:00 AM–4:00 PM or 9:00am –5:00pm. The position requires a minimum commitment of four shifts per month and is based at NYP Westchester in Bronxville, conveniently located directly across from the Metro-North station. Preferred Criteria 2 years' of case management/care coordination experience in an inpatient, HMO, insurance, or other outpatient setting 1 year of leadership experience in a healthcare setting Master's Degree in healthcare related field Required Criteria Bachelors degree in Nursing (BSN) Current NYS Registered Nurse (RN) licensure 3 to 5 years of health care experience in an acute care setting Superior communication, demonstrated problem solving skills, strong business acumen, and interpersonal skills are critical Join a healthcare system where employee engagement is at an all-time high.
