Position Summary:
The Speech Language Pathologist (SLP) provides high-quality diagnostic and therapeutic services to residents with speech, language, cognitive-communication, voice, and swallowing disorders. This role partners closely with nursing, dietary, and other interdisciplinary team members to create and implement individualized treatment plans that improve communication skills and quality of life.

Key Responsibilities
Assessment & Evaluation
  • Conduct comprehensive evaluations of residents with speech, language, cognitive, and swallowing concerns.
  • Develop and maintain individualized treatment plans and measurable goals based on resident needs.
Therapeutic Intervention
  • Provide evidence-based speech, language, voice, and swallowing therapy to residents in one-on-one or group settings.
  • Monitor and document progress, adjusting treatment plans as necessary.
  • Educate residents, families, and staff on strategies to support communication and safe swallowing.
Collaboration & Documentation
  • Work closely with physicians, nurses, dietitians, and other rehab professionals to ensure coordinated care.
  • Maintain accurate, timely, and compliant clinical documentation and billing records.
  • Participate in interdisciplinary team meetings and care plan reviews.
Compliance & Professional Development
  • Adhere to all federal, state, and facility regulations, including HIPAA and infection control policies.
  • Stay current with best practices and maintain licensure and continuing education requirements.
Qualifications
  • Master’s degree in Speech-Language Pathology from an accredited program.
  • Current state license (or eligibility) as a Speech Language Pathologist.
  • Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP) from ASHA preferred.
  • Experience in a skilled nursing facility, hospital, or rehabilitation setting preferred.
  • Strong interpersonal, communication, and documentation skills.
