This position is located in a transitional residential home for youth ages 16–18 who have prior involvement with the juvenile detention system.

Duties
- Conduct thorough assessments of youth needs, including mental health, addiction, and housing stability.
- Develop personalized care plans that incorporate crisis management strategies and motivational interviewing techniques.
- Provide crisis intervention when urgent issues arise, ensuring immediate safety and stabilization.
- Offer ongoing mentoring and behavioral health support to promote positive behavioral change and resilience.
- Monitor youth progress regularly, adjusting care plans as needed while maintaining detailed documentation of interactions.
- Collaborate with multidisciplinary teams, including OJJ, healthcare providers, and community organizations to coordinate seamless service delivery.
Skills
- Proven experience in crisis intervention and crisis management with the ability to remain calm under pressure.
- Strong background in addiction counseling, behavioral health, or social work with a focus on vulnerable populations.
- Skilled in motivational interviewing techniques to foster client engagement and motivation for change.
- Supervising experience is preferred to guide junior staff or volunteers effectively.
- Knowledge of child welfare systems, Medicare policies, and workforce development initiatives.
- Excellent communication skills with the ability to build trust and rapport with youth with diverse backgrounds.
