What will you be doing:

 

The Palliative Care Nurse Practitioner is a registered professional nurse with advanced graduate education and clinical training who provides consultative palliative care services to patients with advanced illness through the evaluation and assessment of the health history, addressing disease prognostication, assists with advanced care planning, provides recommendations for palliation of symptoms in collaboration with patients, families and the attending provider. This is a Per Diem position that covers the south shore area.

Compensation: 62/hr. - 68/hr. Commensurate with experience

What we’re looking for in you:

 

  • Completes history and physical examinations, evaluates the patient's medical condition and health history, interpreting results from laboratory and diagnostic tests or procedures, and documents the findings in the patient's medical record.
  • Provide palliative consults for those with serious illness, comfortable with providing candid goals of care discussions, assist with advanced care planning, make recommendations for symptom management to attending provider and coordinating hospice care services as appropriate.
  • Reviews clinical information, coordinates patient care in consultation with the palliative care interdisciplinary team and other healthcare practitioners.
  • Reviews protocols for treatment and proposes options for interventions based on the current medical literature.
  • Consults the collaborating physician or organizational leadership when the patient's plan of care is outside standardized practice and protocols.
  • Graduation from an accredited School of Nursing and accredited Nurse Practitioner Program.
  • Current nursing licensure in State and CPR certification, as required by organizational policy.
  • Master's or doctoral degree with a minimum of one (1) year Nurse Practitioner experience who has experience in geriatrics, oncology, palliative care and/or hospice
  • Accreditation from an approved certifying body for advanced practice nursing as required by State.
  • Certification in a specialist area preferred; (e.g. ACHPN, AOCN, AWOCN, etc.). If not ACHPN certified, will devise a plan to obtain certification within 18 months of hire.
  • Excellent observation, verbal and written communication skill, problem solving skills, mathematical skills; nursing skills per competency checklist.
