Role Description
We offer full-time, part-time, or prn opportunities nationwide. The Consultant Dietitian will provide comprehensive nutrition services for residents in long-term care facilities. Responsibilities include conducting detailed nutrition assessments, identifying and managing malnutrition or risk of malnutrition, and developing individualized care plans in accordance with regulatory standards.
The Consultant Dietitian will work closely with interdisciplinary teams to ensure coordinated, high-quality nutrition care and compliance with MDS documentation and other regulatory requirements. This role is ideal for a dietitian with experience in long-term care who is committed to improving resident health outcomes through evidence-based, person-centered nutrition practices.
Qualifications
- Document all nutrition care using the Nutrition Care Process within the facility’s EMR system, including high-risk residents, and make tailored nutrition recommendations accordingly.
- Conduct nutrition assessments, screen for malnutrition, and develop individualized care plans, including MDS Section K and related documentation.
- Provide medical nutrition therapy and support for a variety of conditions common in older adult and post-acute populations.
- Collaborate effectively with interdisciplinary teams to ensure coordinated, high-quality nutrition care.
- Adapt nutrition care strategies to meet the specific needs and regulatory requirements of long-term care facilities.
- Maintain RD credentials and applicable state licensure, keeping current with best practices and regulatory standards.
- Previous experience in long-term care preferred but not required.
