We are seeking a compassionate, clinically focused Registered Dietitian to join our team at a skilled nursing and rehabilitation center in West Philadelphia. This position plays a vital role in enhancing the health, recovery, and quality of life of our residents through individualized nutrition care, interdisciplinary collaboration, and regulatory compliance.

Key Responsibilities
Clinical Nutrition Management
- Complete comprehensive nutrition assessments for new admissions, high-risk residents, and annual reviews.
- Develop, implement, and monitor individualized nutrition care plans.
- Conduct routine and follow-up assessments in accordance with federal, state, and facility requirements.
- Monitor residents for nutritional risk factors including:
- Significant weight changes
- Wounds and pressure injuries
- Malnutrition
- Dysphagia
- Diabetes management
- Renal disease
- Enteral nutrition support
- Recommend appropriate nutrition interventions and therapeutic diets.
Interdisciplinary Collaboration
- Participate in care plan meetings and interdisciplinary team discussions.
- Collaborate with physicians, nursing staff, speech therapists, rehabilitation teams, and food service personnel to ensure optimal resident outcomes.
- Provide nutrition education to residents, families, and staff as needed.
- Support discharge planning and transitional care initiatives.
Regulatory Compliance & Quality Assurance
- Ensure compliance with all applicable federal, state, local, and facility regulations, including CMS and Department of Health requirements.
- Maintain accurate and timely documentation within the electronic medical record.
- Participate in Quality Assurance and Performance Improvement (QAPI) activities.
- Assist during surveys and inspections, responding to nutrition-related inquiries and observations.
Food Service Collaboration
- Review menus for nutritional adequacy and regulatory compliance.
- Assist with therapeutic diet implementation and menu modifications.
- Conduct meal rounds and evaluate resident satisfaction with food quality and service.
- Work closely with culinary and dietary staff to ensure resident preferences and nutritional needs are met.
Qualifications:

Required
- Registered Dietitian (RD/RDN) credential through the Commission on Dietetic Registration (CDR).
- Licensed Dietitian-Nutritionist (LDN) in Pennsylvania or eligible for licensure.
- Bachelor's degree in Nutrition, Dietetics, or related field.
- Strong knowledge of medical nutrition therapy and long-term care regulations.
- Excellent communication, organizational, and documentation skills.
Preferred
- Minimum 1–2 years of experience in skilled nursing, rehabilitation, long-term care, or healthcare nutrition.
- Experience with electronic medical records.
- Knowledge of CMS regulations, MDS processes, and survey preparedness.
