We are seeking a passionate and dedicated Patient Care Coordinator to join our Phoenix AZ team! Hours of operation are Monday-Friday 8:30am-5pm.
SUMMARY
The Patient Care Coordinator attends to the overall day-to-day needs of consumers in a timely and efficient manner. Maintains safe and organized delivery system for consumers, including special requests. Handles all communication between consumers and Nutrishare.
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned. Interfaces with consumers and records interview information into computer for customer service by performing the following duties:
- Talks with consumers by answering and directing all incoming phone calls.
- Receives supply/prescription orders/refills per consumer requests.
- Maintains and updates supply sheets via website and provides copies to consumers upon request.
- Responds to and documents all consumer questions, compliments and complaints using the Nutrishare Occurrence Report online.
- Maintains consumer travel logs.
- Communicates changes in compounding schedule with clean room technicians and Pharmacist.
- Communicates necessary consumer information to appropriate individuals.
- Generates delivery tickets, confirms delivery tickets and corrects backorders and maintains file system for all tickets.
- Maintains patient demographics in CPR
- Assists Pharmacist, as needed, in preparation and dispensing of prescriptions, and other duties.
- May be cross trained in other duties for “Pharmacy Technician” and/or “Purchaser”.
- Other duties may be assigned at any time via verbal or written instructions.
- Ability to get along and work effectively with others.
- Regular, predictable attendance is required.
SUPERVISORY RESPONSIBILITIES
This job has no supervisory responsibilities.

EDUCATION and/or EXPERIENCE
Associate's degree (A. A.) or equivalent from two-year college or technical school; or six months to one-year related experience and/or training; or equivalent combination of education and experience; Pharmacy Technician License required
LANGUAGE SKILLS
Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports and correspondence. Ability to speak effectively before groups of customers or employees of the organization.
MATHEMATICAL SKILLS
Ability to calculate figures and amounts such as discounts, interest, commissions, proportions, percentages, area, circumferences, and volume. Ability to apply concepts of basic algebra and geometry.
REASONING ABILITY
Ability to understanding to carry out instructions furnished in written, oral, or diagram form. Ability to deal with problems involving several concrete variables in standardized situations.
CERTIFICATES, LICENSES, REGISTRATIONS
Pharmacy technician license license is required. PTCB required
