Passionate about patient care and wound management? We are looking for a dedicated Wound Care Nurse Practitioner to care for residents in Skilled Nursing Facilities across California, working closely with facility teams to improve outcomes.

What You'll Do:
- Assess and manage acute and chronic wounds
- Develop and implement personalized care plans
- Monitor progress and adjust treatments as needed
- Document status and care for new admissions
What We’re Looking For:
- Current CA State RN License and Licensed Nurse Practitioner.
- A minimum of 2 years’ experience in wound care, with a solid understanding of treatment protocols and best practices.
- EMR proficiency; SNF experience and Wound Care Certification a plus
