Duties

- Provide direct patient care in home health environments.
- Utilize medical terminology effectively to communicate with healthcare professionals and document patient progress.
- Perform therapeutic exercises and modalities such as trigger point therapy to aid in pain relief and recovery.
- Monitor patients' responses to treatment and report any changes or concerns to the supervising therapist.
- Educate patients and their families on exercises, home care techniques, and lifestyle modifications to support ongoing recovery.
- Maintain accurate records of patient progress and treatment outcomes in compliance with facility policies.
Skills
- Strong knowledge of medical terminology relevant to physical therapy practices.
- Experience or familiarity with geriatric care, particularly in acute care settings.
- Proficiency in providing compassionate patient care across various environments including inpatient, outpatient, skilled nursing facilities, and home health services.
- Ability to perform trigger point therapy techniques effectively.
- Excellent communication skills for interacting with patients, families, and healthcare team members.
- Strong organizational skills to manage multiple patients’ needs efficiently while maintaining attention to detail.
- A commitment to ongoing professional development within the field of physical therapy.
- Home Health exprience preferred.
