Job Overview
We are seeking a dedicated and compassionate Physical Therapist (PT) to join our healthcare team. Responsibilities
- Develop and implement personalized care plans based on patient assessments and goals.
- Administer trigger point therapy and other therapeutic interventions to alleviate pain and improve mobility.
- Collaborate with healthcare professionals to coordinate comprehensive patient care in hospital and home community settings.
- Educate patients and their families about treatment plans, exercises, and self-care strategies.
- Monitor patient progress and adjust treatment plans as necessary to ensure optimal outcomes.
- Maintain accurate documentation of patient evaluations, treatments, and progress notes using medical terminology.
Skills
- Strong knowledge of care plans, physiology, anatomy, and medical terminology.
- Experience in acute care settings, home health services, or orthopedic rehabilitation is preferred.
- Ability to perform trigger point therapy effectively.
- Excellent communication skills for interacting with patients, families, and interdisciplinary teams.
- Compassionate approach to patient care with a focus on improving quality of life.
- Strong organizational skills to manage multiple patient cases efficiently.
- Home Health experience preferred.
