Job Overview
We are seeking a dedicated and compassionate Physical Therapist (PT) to join our healthcare team. The ideal candidate will have a strong background in patient care, with the ability to develop and implement effective care plans tailored to individual patient needs. This role involves working closely with patients in home health, environments. The PT will utilize their knowledge of physiology and anatomy to support patients in their recovery journey.

Responsibilities

• Develop and implement personalized care plans based on patient assessments and goals.
• Administer trigger point therapy and other therapeutic interventions to alleviate pain and improve mobility.
• Collaborate with healthcare professionals to coordinate comprehensive patient care in hospital and home community settings.
• Educate patients and their families about treatment plans, exercises, and self-care strategies.
• Monitor patient progress and adjust treatment plans as necessary to ensure optimal outcomes.
• Maintain accurate documentation of patient evaluations, treatments, and progress notes using medical terminology.

Skills

• Strong knowledge of care plans, physiology, anatomy, and medical terminology.
• Experience in acute care settings, home health services, or orthopedic rehabilitation is preferred.
• Ability to perform trigger point therapy effectively.
• Excellent communication skills for interacting with patients, families, and interdisciplinary teams.
• Compassionate approach to patient care with a focus on improving quality of life.
• Strong organizational skills to manage multiple patient cases efficiently.
• Home Health experience preferred.
