Responsibilities
- Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support.
- Monitor and record vital signs such as blood pressure, pulse, temperature, and respiration to ensure patient health status.
- Support medication administration following prescribed protocols and document all medical records accurately using Electronic Health Record (EHR) systems.
- Provide specialized care for patients with dementia, Alzheimer's disease, or other memory impairments through tailored memory care techniques.
- Assist with catheter care, wound care, and other medical procedures under the supervision of healthcare professionals.
- Support patients during hospice or palliative care by offering emotional comfort and managing symptoms effectively.
- Maintain a clean and safe environment by assisting with light housekeeping tasks such as laundry and cleaning, ensuring compliance with HIPAA regulations for patient privacy.
Skills
- Proven experience in home health or long-term care environments with knowledge of medical terminology and documentation practices.
- Strong patient observation skills to detect changes in condition or behavior promptly.
- Familiarity with EMR (Electronic Medical Records) and EHR systems for accurate record keeping.
- Ability to assist with complex medical needs including medication administration, Hoyer lift operation, and vital signs measurement.
- Experience working with individuals with developmental disabilities or disabilities requiring specialized caregiving approaches.
- Knowledge of Alzheimer’s care, dementia care, hospice support, and memory care techniques.
- CPR certification and training in catheter care, hospice support, and other relevant medical procedures preferred.
- Excellent communication skills to collaborate effectively with healthcare teams and family members.
